HSBC add on

Does anybody know the process of add on card for hsbc. I want to get one but not sure about the documents required for it.

SMS the first 2 letters of your Name + Date of Birth (DDMM format) + Last 4 digits of your addon Credit card (ex:MA28101234 ) to +919958418884 to get your PIN

My add-on tap&pay stopped suddenly out if nowhere and now pin sms not coming
