However the card was already expired. Next day I received a message that the forex load was unsuccessful and the transaction reversed on 28th March.
I am thinking of filing a RBIO complaint for negligence and deficiency in services. How much compensation should I demand?

If you have these proper documentations then you can file complaint

I think you missed a point. I had applied for non personalized card which doesn't come with name embossed on it. Further, I have received a text message to pickup the card from nearest branch. Upon picking up the kit, and opening it, I realised it was expired. Now the kit didn't have any sort of marking which will enable anyone to know the expiry date of card. Whereas other kit which was issued later (separate application) clearly mentions that the card will expire in 2029.If name not written then last 4 digit of card number should match with the letter they sent with card.
First raise a complaint with hdfc customer care if its not resolved then grievance, if not resolved by grievance then pno and if these all takes more than 30 days and even you aren't able to get the solution then you can complain to RBIO.
You can't go directly to RBIO without even raising a complaint with bank

I have already raised the complaint to grievance redressal. They forwarded to the branch and the BM issued a written apology and closed the complaint. I had demanded 20K compensation.
BM was not involved at that time. For first application I got the card from an executive. Activated the card as instructed in the mail. Subsequently received SMS that forex card can not be loaded after that forex load has failed and transaction has been reversed.Did he issued you a new card or asking to reapply?
not a third party agent....after complaint I got a call from hdfc cc department confirming its their employee only.Because of false promises made by 3rd party agents, on call/chat by agents (to meet their sales targets), RBI introduced concept of card activation.
If you have been given false promises by someone for which bank can't be held liable, the only solution for such people is not to activate the wrong card sent.

I had to apply for a second time and that time I got a valid card and the load was successful
