Hdfc Credit card reward redemption by submitting Form in Branch..need guidance and share experiance

Hello friends
i do not have hdfc bank account and i hold its Bharat Rupay Credit Card and have 1000 points in it.
i got mail for millenia upgrade but want to redeem its points before upgrade.
so i have uploaded its rewards catalogue and rewards redemption form.

in the rewards catalogue i think croma vouchers would give best value and easily usable.

how to get it croma vouchers ....what to write in rewards redemption form.
