NEED HELP REGARDING HDFC BANK CREDIT CARD UPGRADE

Hello Community Members,

I am seeking assistance regarding an upgrade for my HDFC credit card.

Currently, I hold a Paytm HDFC co-branded credit card, which I've had for two years. I recently emailed HDFC to request an upgrade to the Millennia card. Initially, they denied my request, but after further discussion, they provided me with a form to complete. However, I’m facing a few challenges:

Income Proof: I don’t currently have income proof available, though one of my SBI card has a credit limit of 1.58 lakh. Will proceeding with the upgrade via the "credit for card" (C4C) method be acceptable in this case?

Form Submission: I’m unsure about the correct process to submit the form. Should I print it out, complete it by hand, and then scan it as a PDF? Should I also include my recent credit card statement with the submission?

Any guidance on these issues would be greatly appreciated. I’ve attached a screenshot of my conversation with HDFC along with the PDF form for reference.

No need to provide any income proof or card statement since they already have the approval.
Just fill all other details and leave income docs section empty.

1 give them 4 digits
2 MILLENIA (in which you want to upgrade)
3 pricing means you have to write that you are aware of the charges of the card the membership fee
4 coversion of reward points that is crediting of whatever reward points you have according to the variant into which you are upgrading i this is 15% for money back and 30 % for regalia first percent (30% percent of cuurent rewards points will credit into new millenia card)
5 they need to access your cibil report and make and enquiry(cibil hit): give confirmation in YES
