Sure. I have mab 1L elite executive account. Went to branch today, asked for addon, they gave me this form (

I asked them that there is no mention of select and they assured that i will get select card and they manually wrote "rupay-select" on that. I also asked if it can be on my name and i can apply for both add on on this form and they said no problem. I remember writing in similar form acc open as well, it didn't option for rupay select but they said i will get it and i got the same. I hope the same happens here as well
