You have to first join club avolta through your Gmail or Facebook account.

Once account is made, Click above link and put requested details. Then you will be asked to match status with airline/hotel elite status. Whatever you chose, you will get various options. Chose the program where you hold the top elite membership. It should be in the option menu.

Then upload screeshot of your other program membership card and submit. Within few hours you will get decision by email.
