That's bad. Did they change any policy now? If your net salary is more than 3L, you don't need to maintain anything else. They will give you premier salary account and premier credit card. Am I missing something?

Yes, first ask them to change salary account and yes, you need to write the code and give them in written. Keep aadhaar card (attested) copies with you and take a picture of the form before submitting for future proof.
