Can someone please confirm if domestic lounge guest access on M4B card requires the card owner to be present?
If not, how can one give guest access to a family members?
Usually cardholder should be physically present ,it depends on the receptionist but mostly it mandatory

Otherwise give add on card to your family members in their name

It is mandatory for the cardholder to be present. Most lounges grant access to guests by using someone else's spare cards with FREE lounge access rather than paying for the guest. Needless to say (or maybe it is actually needed) this is against the conditions of their partnerships/deals ie. Priority Pass and the banks themselves. The lounges do it because it's a lot of additional money for them and anyway most people wouldn't pay for a lounge (the quality is usually pretty bad in most domestic lounges anyhow, moreso with the overcrowding), and lounges found their gold mine in abusing and breaching their contractual terms to enrich themselves and also pleases the common user who wants free buffet for their whole family.

But things are quite different overseas, i had to use my Magnus Priority Pass Guest visit to get one of my family members into Turkish Lounge at Miami and Marhaba Lounge at Dubai last year ,the lounge agent wouldn't budge on putting a visit on my RBL World Safari Priority Pass, they match name on card and boarding pass very strictly

Not sure about RBL but no Indian bank tends to have direct partnerships with any lounges outside India so you need to use the Priority Pass/Lounge Key/etc that came with your card for any international lounge usage.

The exception is some multinational banks which already have a long standing relationship with larger chains like Plaza Premium and sometimes they extend the partnership across several countries where the bank operates and accept their card as a way to access their lounge rather than something like Priority Pass/Lounge Key which is more expensive for them to operate.

But I am not aware of any card like this in India for international lounge access except for AMEX with their own lounge, the Escape lounges and Delta lounges when flying Delta.

Yes, quite true for international lounges. Even I had to show Priority Pass and they verified it with my ID (Passport) and boarding pass.
For domestic iv never actually used a guest access and I usually end up at the airport just in time to board so I skip the lounges altogether.
