Fill floater form, attach latest Infinia statement, and kyc docs. Send to Chennai address and email a copy to grievance.redressalcc@hdfcbank.com as well.
If no update in a week, complaint against RM and BM over same email.
Bank RM helped me,
they took signature of few forms and rest everything processed at their end,
if one branch RM denied try going another branch you will get it.
