I was able to merge my limits in case of HDFC (Infinia & Regalia), but not sure about the process with ICICI bank.
This is not at all true. I have received EPM last week purely based on net salary criteria. No conditions attached. Just called random RM from Icici credit card team and they helped in getting the card within 3 business days.

Finally, I am able to get an Add-on card for my Magnus Burgundy Card as well!! This required special approval from Branch head to the operations team and within 2 working days I can now see the add on card in my mobile banking app! :D
